Recurrent Pituitary Neuroendocrine Tumors: Anatomical Predictors of Gross-Total Resection with a Proposed Pituitary Resectability Score

A prospective study proposing an anatomical score to predict gross-total resection in recurrent pituitary neuroendocrine tumors.

Zohney, Abd Elshafouk, Aziz, Ismail and El-Bahy evaluated 32 patients undergoing endoscopic endonasal reoperation. The proposed score incorporates tumor size, morphology, cavernous sinus invasion and extrasellar extension.

Gross-total resection was achieved in 91% of class I tumors and 57% of class II tumors, while no gross-total resections were achieved in classes III–IV. The score remains preliminary and requires external validation.

Why it matters

Recurrent PitNETs are often substantially more difficult than primary tumors. A structured anatomical score may help with preoperative counseling, operative planning and realistic assessment of resectability, particularly in revision endoscopic surgery.
